  • If applicable: Do I have different or additional coverage a diabetes diagnosis? (same procedure codes)

2. How many visits are covered per year?

  • $240 for Initial sessions (90 minutes)

  • $175 for follow up sessions (50 minutes)

Group Support

  • $300 for each 6 week series

  • $50 per session for on going groups
